We understand the importance of timely placements and clear communication, and we work closely with local authorities, social workers, and commissioning teams to ensure that every referral is handledwith care and professionalism
Step-by-Step Referral Guide
1. Initial Enquiry
- Local authorities or social workers can contact us directly by phone or email.
 
- We will provide an overview of our current vacancies, the needs we can meet, and our Statement of Purpose
 
2. Referral Submission
- A referral form and supporting documents (care plan, risk assessments, education and health information, SEN/EHCP if applicable) should be submitted.
 
- This allows us to build a clear picture of the child or young person’s needs
 
3. Assessment & Matching
- Our management team reviews the referral against the needs of the child and the skills of our staff team.
 
- We  consider  compatibility  with  existing  residents,  therapeutic  needs,  and  the  ability  to provide safe, stable care
 
- If required, we may request additional information or hold a professionals’ meeting
 
4. Decision & Placement Offer
- If  we  can  meet  the  child’s  needs,  we  will  confirm  availability  and  issue  a  formal placement offer.
 
- If  we  cannot  meet  the  needs,  we  will  provide  aclear  explanation  and,  where  possible, signpost to alternative provision
 
5. Pre-Placement Planning
- A  transition  plan  is  agreed  with  the  placing  authority,  which  may  include  visits, introductions, and phased moves where appropriate
 
- Risk assessments, careplanning, and safeguarding measures are finalised.
 
6. Admission & Welcome
- On  admission  day,  the child  or  young  person  is  welcomed  into  their  new  home  by  staff and peers
 
- They   receive   a  child-friendly   welcome  pack,  and  their  care  plan  is  implemented immediately
 
7. Ongoing Partnership
- We   provide   regular   updates,   statutory   reports   (Regulation   44   and   45),   and   open communication with the placing authority
 
- Reviews and meetings are scheduled in line with statutory requirements and the child’s
needs
